WHO IS PART OF YOUR PRIMARILY HOME TEAM?
Your Primarily Home care team consists of a licensed physician and/or advanced practice provider (nurse practitioner or physician assistant), registered nurse, social worker, pharmacist, and community health worker, who will provide you the support and guidance you need.
WHAT TO EXPECT?
After your in-home visit, the information gathered will be shared with your primary care provider and the best care plan will be developed with you to fit your lifestyle and daily needs. The Primarily Home care team is available to answer questions and recommend solutions for any health-related concerns you might have. Your HCA Healthcare primary care provider will stay in close contact with your Primarily Home care team as we collaborate to manage your healthcare needs.
